The Forms & Surveys builder lets you create custom forms, questionnaires, and surveys that can be shared via link or QR code. Forms collect responses from anyone — volunteers, members of the public, event attendees, or partners.
Forms are for general-purpose data collection only. Do not use them for volunteer applications — the volunteer application has its own dedicated builder configured separately.
Creating a form
Navigate to Forms & Surveys from the admin menu
Click Create Form
Add a title, description, and optional success message
Build the form using the drag-and-drop editor
Field types
The form builder supports the following field types:

Repeatable Group — a section the respondent can duplicate (e.g. adding details for multiple people)
You can also add Headings and Content blocks (rich text with images) to structure the form.
Each field can be marked as required or optional, and has an optional description shown as helper text.
