Creating custom forms and surveys with the form builder

The Forms & Surveys builder lets you create custom forms, questionnaires, and surveys that can be shared via link or QR code. Forms collect responses from anyone — volunteers, members of the public, event attendees, or partners.

Forms are for general-purpose data collection only. Do not use them for volunteer applications — the volunteer application has its own dedicated builder configured separately.

Creating a form

  1. Navigate to Forms & Surveys from the admin menu

  2. Click Create Form

  3. Add a title, description, and optional success message

  4. Build the form using the drag-and-drop editor

Field types

The form builder supports the following field types:

  • Text — short text input for names, single-line answers

  • Long Text — multi-line text area for detailed responses

  • Number — numeric input

  • Date — date picker

  • Time — time picker

  • Phone — phone number with validation

  • Email — email address with validation

  • Yes / No — a simple toggle for binary questions

  • Dropdown — single selection from a list of options

  • Multiple Choice — radio buttons for single selection

  • Checkboxes — tick one or more options from a list

  • Rating — star-style rating

  • Matrix — grid-style question with rows and columns

  • File Upload — let respondents attach files

  • Repeatable Group — a section the respondent can duplicate (e.g. adding details for multiple people)

You can also add Headings and Content blocks (rich text with images) to structure the form.

Each field can be marked as required or optional, and has an optional description shown as helper text.

Form settings

Allow multiple submissions — let the same person submit more than once.

Close form on date — set an automatic close date with a custom "form closed" message.

Redirect URL — send respondents to a specific page after they submit.

Public or private — public forms can be listed; private forms are accessible by link only.

Distributing a form

Share your form in two ways:

  • Link — copy the shareable URL and send it via email, social media, or your website

  • QR code — generate a printable QR code with your organisation's logo embedded

Permissions

Only Admins can create and manage forms.
