Managing Incident Reports

Reviewing, editing and closing incident reports submitted by volunteers

When a volunteer submits an incident report, it appears in Incident Management for your team to review. Navigate to Incident Management from the main menu to see all reported incidents.

The Incident Management page

The page has a list of incidents on the left with filters, and a detail view on the right. Each incident shows the date, activity, severity (colour-coded) and current status.

Filters

Narrow down the list by:

  • Program — incidents for a specific program

  • Severity — filter by severity level

  • Activity type — incidents during an activity vs outside of one

  • Day of week — incidents on specific days

  • Search — search incident descriptions

Incident statuses

Each incident moves through a workflow:

  • New — just submitted, not yet reviewed

  • In Review — being looked at by a manager

  • Completed — reviewed and resolved

  • Archived — filed away for records

Change the status using the dropdown in the incident detail view.

Viewing incident details

Click any incident to see the full report:

  • Severity and category (the type of incident)

  • Location — the activity or address where it happened

  • Date and time

  • People involved — volunteers and non-volunteers

  • Description and further action notes

  • Photos — any images the reporter attached

  • Submitted by — who reported it and when

Editing an incident

Click the options menu (three dots) in the top right of the detail view and select Edit. You can update the description, change the severity level, add notes or attach additional information. All edits are tracked in the edit history.

Comments

Add internal comments and attachments to an incident for discussion between managers. Comments are not visible to the volunteer who submitted the report.

Exporting

Export an individual incident to CSV from the options menu, or export multiple incidents in bulk from the incident list.

Closing an incident

Change the status to Completed and add any closing comments. The incident remains in your history and can be reopened if needed.

Permissions

  • Program Managers can view and manage incidents for activities in their programs

  • Admins can view and manage all incidents across the organisation

  • Activity Leaders can view incidents for their activities
